We have an exciting opportunity to join our team as a Palliative Nurse Practitioner.
In this role, the successful candidate Provision, coordination, and assurance of comprehensive nursing care using nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of therapeutic, corrective, and prescriptive measures of the designated inpatient or outpatient patient population within a collaborative practice agreement established with a specific physician on the staff of NYU Hospitals Center
Screens patients referred to the Palliative Care Consultation Team for needed services in the areas of advanced care planning, symptom management, extended care options, spiritual, emotional and bereavement support.
Performs directed health history, physical examination and psychosocial assessment specific to patients referred to palliative care.
Works collaboratively with palliative care consultation team attending physicians, consulting physician(s), and other disciplines to identify, develop, implement, and communicate an appropriate plan of care that considers potential outcomes and goals of care, maximizes patient/family preference, enhances quality, access, and cost-effective outcomes.
Prescribes medications for palliative care patients according to NYS law, including controlled substances with DEA authorization.
Orders laboratory, radiological and diagnostic studies as indicated, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include adjustment in plan of care or discussion with Medical Director of Palliative Care or attending MD.
Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol.
Evaluates patient responses to treatment and therapy throughout the continuum of care.
Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols.
Supports service and unit level leadership by providing clinical consultation and resource assistance for patients with life threatening or deteriorating chronic illness, as requested and as identified.
Oversees appropriate placement of palliative patients, by working collaboratively with the Medical Directors and/or Service Chiefs, Nurse Managers, Advanced Practice Nurses, Attending MDs, house staff, and Patient Access Department.
Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and by laws as outlined in the delineation of privileges.
Acts as a professional role model to all levels of staff by incorporating expertise and related experiences into all aspects of patient care.
Articulates and demonstrates nursing process when exercising responsibilities, and in providing support and guidance to staff.
Articulates, uses, and participates in developing process standards in area of expertise.
Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.
Coordinates inpatient hospice placement at NYULMC with onsite hospice RNs and other team members.
II. COORDINATION, LEADERSHIP & CARE MANAGEMENT
Supports the mission, philosophy, standards, goals and objectives of the institution, clinical departments, nursing, and Palliative Care Program.
Coordinates the Palliative Care Program, in collaboration with the Medical Director, the Consultation Team and Director of Advanced Practice Nurses
Interacts with other departments to ensure the seamless transitions of care through collaborative communication.
Demonstrates knowledge of the organization's core values and incorporates them into the performance of duties.

III. EDUCATION & DEVELOPMENT

Develops and implements a professional development plan.
Maintains current expertise in area of practice.
Contributes to the development of nursing knowledge in area of practice through writing, research, and formal/informal presentations.
Serves as a clinical expert and resource for the education of peers and other health professionals.
Educates patient and family to facilitate and promote healthy adjustment to new or changed expected outcomes.
Participates in the Patient /Family Education Council.
Assists in the provision of interdisciplinary education programs pertinent to this patient population in collaboration with the Palliative Care Consultation Team.
Assists in the development and delivery of community education regarding advance care planning, and related topics.
Licensure: Current registration as Registered Professional Nurse and Nurse Practitioner in NY State
Education: Master's and/or Doctorate degree in nursing
Certifications: Board certification as Nurse Practitioner required. Palliative Care Certification required or attained within 1 year of hire.
Experience: Minimum of five years of clinical practice experience, preferably in critical care, palliative care, hospice, pain Management and/or Oncology. Nurse Practitioner experience preferred.
Competencies: Excellent communication skills, both written and verbal; Demonstrated ability to interface effectively with other disciplines and outside agencies for collaborative outcomes; previously demonstrated ability to facilitate group processes and work cohesively and collaboratively with members of the interdisciplinary team. Core Nurse Practitioner and advanced clinical competencies outlined under requested privileges. Evidence of ability to manage data and use computer applications.
Membership in related professional associations, e.g., Hospice and Palliative Nurses Association (HPNA) and American Association of Hospice and Palliative Medicine (AAHPN).
Physical stamina, manual dexterity, visual and aural acuity necessary to perform responsibilities.
