Swedish Shared Services is looking for a Patient Safety Officer (1.0 FTE, Day Shift) to work with the Quality and Patient Safety Department at Swedish First Hill in Seattle, WA.
The Patient Safety Officer is responsible for being a member of Swedish organizational response to quality variances and having or developing skills to evaluate variance reports, perform root cause analysis and Healthcare Failure Mode Effect Analysis and investigations for serious safety events in support of the patient safety program. Participate and lead quality and safety meetings throughout the organization. Review and analyze safety trends and help guide strategic planning and prioritization for quality and safety related initiatives.
In this position you will: • Identify high-risk areas through Root Cause Analysis (RCA), case investigations, e-QVR referrals, phone referrals, incidents/accidents, and other relevant data sources. • Conduct interviews with physicians, independent practitioners, managers, clinical nurse specialists, and staff in order to identify case specific details and to develop a complete understanding of events. • Review records, researches cases, and creates formal documentation of adverse events and sentinel events. • Promote integration of programs with Risk Management, Accreditation and Safety, Medical Staff Performance Improvement, and Patient Relations departments. • Educate physicians and managers related to the RCA process and promotes compliance with organizational policies and procedures. • Create and participates in curriculum design for New Employee Orientation and Annual Education.
Qualifications: Required qualifications for this position include: • Bachelor of Science degree in Nursing • Current license to practice as a Registered Nurse in the State of Washington • Two (2) plus years of clinical experience • Project management • Experience in change management with internal & external customers including physicians, staff, managers, and directors • Must have working knowledge of personal computers using email and Microsoft office suite • Flexibility, prioritization and time management skills
Preferred qualifications for this position include: • Process improvement experience and training • Intermediate to advanced proficiency in Windows environment, MS Word, Excel, PowerPoint, Access, Internet Explorer, email and general PC data entry • Operation of routine office equipment (telephone, fax, voicemail, photocopiers, printers, etc.) • Must be familiar with basics of patient safety in a healthcare environment • Seasoned in adult education principles in assessment, planning, implementation and evaluation of program and educational activities
