Position Summary: We have an exciting opportunity to join our team as a Pharmacy Clinical Manager.
In this role, the successful candidate The Clinical Pharmacy Manager will oversee the day-to-day professional practice responsibilities in the areas of Adult Transplant Services by systematically facilitating clinical practice, education, guidance, standard methodologies, workflow, best practice and guidelines in these areas. Additionally, he/she will mentor, coach and perform evaluations for assigned staff.

Promotes a positive and professional image for pharmacy by advocating and demonstrating exemplary teamwork and leadership.
Participates and acts as the inter and intra department liaison in collaborations with nursing, ancillary department directors and medical staff.
Develop, maintain and oversee a high-level inpatient practice.
Participate in multidisciplinary bedside rounds.
Pharmacotherapeutic, pharmacokinetic and pharmacoeconomics consultation.
Patient admission histories.
Patient medication reconciliation.
Participate in staff educational initiatives.
The clinical pharmacy manager will be the leading expert to address patient-care issues as assigned by patient-care area. Manages staff and personnel actions as assigned to the discipline.
This includes hiring, disciplinary action, firing, and conducting performance appraisals and competency-based assessments for initial and ongoing practice.
Provides oversight for quality assurance (i.e., Epic order verification) for decentralized pharmacy programs to improve quality of care.
Establishes, evaluates and reports productivity and safety reports and measures.
Develop and implement clinical practice guidelines, order sets and pharmacy managed protocols that will enhance pharmaceutical care and medication safety.
Clinical intervention, adverse drug reaction, and medication error documentation.
Hospital committee involvement.
Pharmacy resident precepting and establishing PGYII program in assigned discipline.
Pharmacy student mentoring.
Maintain professional competency.
Minimum Qualifications: To qualify you must have a Doctor of Pharmacy degree from an ACPE accredited School of Pharmacy. New York State Pharmacist Licensure. PGY1 Pharmacy Residency training; and PGY2 Pharmacy Residency training or equivalent salaried professorship at a School of Pharmacy. Board certification in Pharmacotherapy or other specialty is required within 18 months of hire. At least 6 months of supervisory experience is preferred. Self-motivation, good communication, organizational, and interpersonal skills..
Required Licenses: Pharmacist License-NYS
