(sm) The Manager of Case Management will effectively lead and supervise a virtual team of 12-15 Field Based Case Managers. This will primarily be a field-based role with a home-based office where you will be spending approximately 50% of the time in the field completing trainings and supervisory visits with staff within an assigned coverage area. Candidates must reside in Miami Dade County with possible travel to Monroe County. Primary Responsibilities:Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating) Case management and coordination of care Takes lead role in setting direction and participating in or developing new programs Sets team direction, resolves problems and provides guidance to members of the team Ensures team meets established performance metrics and performance guarantees Adapts departmental plans and priorities to address business and operational challenges
Required Qualifications: Current, unrestricted RN license in the state of Florida Bachelor's degree in Nursing 3+ years direct managerial experience including but not limited to directly managing the performance management, mentoring / coaching and staffing of employees reporting directly to you in a healthcare or managed care environment 3+ years clinical experience in a long term care environment 3+ years of home health experience or field case management experience in a managed care or healthcare environment Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment Preferred Qualifications: Master's degree in Nursing Bilingual English / Spanish speaking A background in managed care Case Management experience Certified Case Manager (CCM) Knowledge in CMS and State regulations Experience / exposure with discharge planning Experience in utilization review, concurrent review and / or risk managementCareers at UnitedHealthcare Community & State.
