Primary Responsibilities:Work closely with our Community ASAP partners and GSSCs (Geriatric Service Support Coordinators) to implement and comply with our care model and visit requirementsTraining and mentoring staff on our clinical platform to ensure metrics are metProvide excellent customer service to our community partnersWork closely with sales and provider relations to promote and deliver our care modelComply with our governmental contract requirementsCompliance responsibility, coordinate meetings at agencies, handle clinical issues and work with clinical regionAutomobile travel to offices of our contracted providers and to meetings with the same in MassachusettsLocal travel around small geographic region
Required Qualifications:At least 1 year of leadership or supervisory experienceHome care or elder experienceProficient in MS Word, Excel and InternetUndergrad degree in Social Work or BSNAccess to reliable transportation that will enable you to travel to client and / or patient sites within a designated areaPreferred Qualifications:RN or SW with geriatric home care experienceExperience with Community ASAP partners and GSSCs (Geriatric Service Support Coordinators)Careers at UnitedHealthcare Community & State.
