HopeWest is seeking a dynamic leader to join our team of six physicians and two nurse practitioners. The physical environment for HopeWest is unique. Grand Junction is home to our primary campus that includes a beautiful 13 bed inpatient unit, a commercial restaurant, and a coffee shop/counseling center housed in a renovated turn of the century house. Satellite campuses are located in both Montrose and Delta. In addition, we have a retail store in each location, Heirlooms for Hospice which is supported by donations and staffed primarily with volunteers. All proceeds from these stores go to support the mission of HopeWest.

Qualifications and Responsibilities
Board Certified or Board Eligible in Hospice and Palliative Medicine
Colorado Medical License and DEA certification
Effectively works in a team environment
Participates in on-call rotation
Visits patients in their homes, assisted living facility, group home, nursing home and the hospital
Attend weekly interdisciplinary team meetings
Palliative medicine consultations
Shares in hospice administrative duties with HopeWest physicians
Full or part time flexible
