Drafts program description, work plans, annualprogram goals for social services, for approval by CountyCare leadership andoversight committees
Ensures that plans and program goals reflect theneeds of CountyCare membership and quality of care goals and strategicdirections of CountyCare and CCHHS, including NCQA health plan accreditation
Recommends, organizes and manages processes tocomply with regulatory requirements, vendor agreement and best practices
Recommends and oversees social services programs andresources as part of CountyCare's overall service package
Serves as a subject matter expert on community and socialservices and impacts on individual and population health
Performs environmental scans and needs assessmentsincluding but not limited to housing, crisis management, income support,food/nutrition, legal support, peer/consumer advocacy support, communityprograms, outreach/engagement and training and development
Makes recommendations based on assessment findingsconsidering member demographic, geographic and risk stratification variables torecommend priorities for program development
Prepares and monitors program budgets
Monitors performance targets
Draft reports for various audiences
Collaborates and coordinates people and organizations toleverage CountyCare and CCHHS resources
Fosters strong relationships with diverse stakeholdersfrom all community sectors
Engages contracted/delegatedcare management entities and medical management staff, vendors, behavioral andphysical health providers to integrate social service interventions
Facilitatesand promotes timely problem identification, analysis and resolution
Assists individuals who contact CountyCareas needed with immediate needs and linkage to ongoing services
Travels to work sites throughout Cook Countyincluding community agencies, network providers, government and other agencyoffices
Reports to the Director of ClinicalServices
Bachelor'sor higher level degree is required.
Three(3)yearsoftotalexperienceinsocialand/or community services
Programmanagement experience is required.
Must beable to travel to work sites throughout Cook County including communityagencies, network providers, government and other agency offices. Travelthroughout Cook County is required.
Master'sdegree OR other degree as a Doctor of Nursing Practice (DNP),NursingDoctorate (ND), Doctor of Philosophy(PhD), Doctor of Psychology (PsyD), Juris Doctor (JD), Medical Doctor (MD),Doctor of Osteopathy (DO) or Doctor of Public Health (DPH) is preferred.
Project managementcertification is preferred.
Experience within aMedicaid Managed Care Organization ispreferred.
Licensure as a SocialWorker, Case Manager or Counselor in State of Illinois is preferred.
Bilingualin English/Spanish is preferred.
