Primary Responsibilities:Assessment of members to identify unmet needs, coordinate services, community resources and treatment needs Delivering a holistic approach to coordinated care based on the enrollee needs Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file Creating, reviewing and revising Plan of Care plans and focusing on disease management Functioning as an advocate for members and decreasing unnecessary hospitalizations Collaborating with the member's PCP to deliver and coordinate necessary services Building relationships with members and their families; assisting them with proper health choices Ensuring cases are documented in a timely manner
Required Qualifications:Arizona Resident Bachelor's level degree in Social Work or related field with at least 2 years clinical or case management experience with at least 1 of those years being a field based position (i.e.: long-term care, home health, hospice, public health, assisted living) OR minimum of 2 years clinical or case management experience with at least 1 of those years being a field based position with persons who are elderly or persons with physical or developmental disabilities and have been determined to have a Serious Mental IllnessPreferred Qualifications:Bilingual in English / Spanish Licensed Social Worker Medicare / Medicaid experience A background working in geriatric special needs A background in behavioral health Experience with electronic charting Ability to multitask Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook) Careers at UnitedHealthcare Community & State.
