Position Summary: We have an exciting opportunity to join our team as a FGP Staff Phys Therapist(37.5).
In this role, the successful candidate will be responsible for evaluating physical therapy needs and implementing comprehensive, medicallyprescribed physical therapy treatment to assigned patients in a supervises program of rehabilitation.
Responsible for certain level ofpatient care productivity asindicated by individual UnitProcedures. Demonstrates theability to provide expectedpatient contact as indicated byproductivity reports andpatient contact analysis.
Participates in ClinicalEducation of Fieldworkstudents and otherprofessional educationalendeavors. Teaches andsupervises assigned studentsin the appropriate techniques,skills and procedures.Demonstrates a genuineinterest in assisting studentsin enhancing their knowledgeand skill levels. Encouragespositive learning experiencesfor Career OpportunityStudents and other NonFieldwork students.
Completes all required documentation for patient careand reimbursement.Completes and plans allrequired documentation inmedical chart as required byDepartment guidelines andJACHO/CARF requirements.Clinical Documentation isthrough and completes usingappropriate medical andclinical terminology.Accurately records alltreatment given includingcompletion of the dailytreatment record.
Implements Treatment Plansand Protocol according tocurrent professional andhealth system standards.Establishes achievable andappropriate treatment goalswith patient and family.Considers a variety ofvalidated treatmentapproaches and rationales forachievement of patient goals.Selects appropriate technicalaids/assistive devices andcorrectly refers patients toother clinical services toachieve stated program/teamtreatment goals. Assesseseducational needs of patient,families, and implements avariety of methods forpatient/family education andtraining during rehabilitationprocess.
Initial Assessment andEvaluation of Physical Therapyneeds for specifically assignedpatient populations - Reviewsmedical chart to ascertainimportant medical informationand clarifiesreferral/Treatment orders ifnecessary for unfamiliardiagnosis. Selects appropriateassessment tools and methodsin order to accuratelydetermine the patients statusfor developing optimaltreatment plan. Demonstrateseffective clinical decisionmaking and implementschanges/adaptations to theTreatment Plan whenappropriate.
Minimum Qualifications: To qualify you must have a Bachelors Degree in Physical Therapy. New York State Physical Therapy License or temporary permit.Good Communication and interpersonal skills..
Required Licenses: Phys Thrpst License-NYS
Preferred Qualifications: Preferred Qualifications:Masters Degree in Physical Therapy or related field.
