Primary Responsibilities Make outbound calls and receive inbound calls to assess members' current health status Identify gaps or barriers in treatment plans Provide patient education to assist with self-management Make referrals to outside sources Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels This is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes. Additionally, strong organizational skills and multitasking abilities will be keys to success
Required Qualifications Current, unrestricted RN license in the state of Florida5+ years experience as an RNExperience working with clients in the community setting Demonstrate a proficiency in computer skills including Microsoft Word, with the ability to navigate a Windows environment Must have a designated work space and access to install secure high speed Internet via cable / DSL in home Bilingual fluency in English and SpanishPreferred Qualifications: A background in managed care Case management experience Certified Case Manager (CCM) Experience or exposure to discharge planning If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.Careers with UnitedHealthcare.
