Job Summary: A transition liaison position for potential or existing clients of the Anoka Metro Regional Treatment Center (ARMTC), Community Behavioral Health Hospitals (CBHH), Intensive Community Residential Treatment (IRTS), Community Hospitals and/or a Competency Restoration Program (CRP) in Region 5+ (comprised of the Leech Lake Band and Mille Lacs Band of Ojibwe and Aitkin, Cass, Crow Wing, Morrison, Todd, and Wadena counties). Includes supporting Region 5+ in identifying transition options for clients in need of alternative placements or discharge from a formal placement.

Grade 7 begins at approximately $56,800 annually.
a. Bachelor's degree from an accredited college with major in social work or related degree and
b. Five (5) years of experience in social services, and at least three (3) of those years’ experience in a public welfare agency in the specific unit or area of specialty
c. Master’s degree in social work and licensed Independent Clinical Social Worker in the state of Minnesota preferred and three years (3) of experience in a public or private social service agency
d. A combination of social work of related post-secondary and/or professional education and demonstrated relevant work experience described above, equivalent to a total of a minimum of 10 years.
