**Acute Care, Med-Surg, Tele** *6MB - Planned Surgery - Per diem variable positions available. *6EH - Ortho Trauma - Per diem days as well as per diem nights available. *7EH - Trauma/General Surgery - 100% night shift position available; Per diem variable shifts available. *5EH - Med/Surg/Tele - 90% night shift available; Per diem nights available. *4EH - Med/Tele/Oncology - Per diem night positions open. *8EH - Burns/Plastics/Peds - Per diem days/nights available. *4WH - Trauma Rehabilitation - 100% night shift available; Per diem variable shifts open. *3WH - Neurosciences - 90% Night and 90% Day positions open; and, per diem variable position open. *3EH - Med/Tele - Per diem variable shift positions open. *Acute Care Float Pool - Per diem variable shift positions open.
**CCN - Continuity of Care Program** Current openings area: *per diem.
The Continuity of Care Nurse (CCN) coordinates all aspects of care leading to a timely transition to the next level of care for patients on acute care units. The CCN serves as a liaison to patients, families, medical teams, nursing staff, social services, interdisciplinary team members, consulting and referring physicians and agencies involved in the care of patients from admission through the discharge process. The CCN identifies the clinical targets and milestones for discharge and eliminates barriers as needed. The CCN communicates the discharge plan to appropriate team members.Requires min. two years acute care or critical care nursing experience.
Legal Requirement - Current license to practice as a registered nurse in the State of Washington.
