We have an exciting opportunity to join our team as a Nurse Practitioner for Adult Primary Care in Bellevue Hospital.
In this role, the successful candidate will work within one of our continuity firms, providing access for new patients, as well as provide support to existing provider panels by offering same-day availability appointments to patients. Both of these functions would help improve clinic wait times for new and urgent appointments. The work schedule will be 5 days per week including Saturday coverage.
Continuity Firm Provide same-day or walk in access for a minimum of 5 patients with Bellevue PCPs in their Firm who are unavailable Review all ordered laboratory, radiographic and ancillary studies Document medical evaluation in QuadraMed Electronic Health Record (EHR) Communication/coordination of results with PCP and patients Assist with medication refills, patient forms and laboratory re-orders Participate in inter-disciplinary care management of high risk patients Maintain a Saturday clinic each week (NB: Role may be subject to change as the practice needs change)
New Intakes Evaluate a minimum of 4 new patients with focused history and physical examination Order appropriate laboratory, radiographic and ancillary studies, as indicated Make referrals to specialists, as indicated Document evaluations in QuadraMed EHR
Administrative Participate in GIM quality improvement activities Review charts/reports for collaborative care and population health management
To qualify you must have
Licensure: Current registration as a Nurse Practitioner and Registered Professional Nurse in New York
Education: Master's Degree in Nursing; Board certification as an Adult or Family Nurse Practitioner; Basic Life Support.
Required Licenses: Nurse Practitioner-NYS, Registered Nurse License-NYS, DEA
