Position Summary: We have an exciting opportunity to join our team as a Nurse Coordinator.
The RN coordinator for the NYU Langone Center for Stroke and Neurovascular Diseases (CSND) is responsible for the coordination and management of patient care for the center. This will include coordination and implementation of the multidisciplinary patient coordination in collaboration with various HOS and SOM departments. Working with the multidisciplinary team to establish patient care pathways and developing the program. Work with the transdisciplinary team to achieve the mission of research, education and patient care for all aspects of the center. Work in collaboration with all leadership to ensure quality, growth, and customer satisfaction.
Clinical coordination plans for and implements the CSND services and programs which will include the coordination of patient care (testing, procedures and clinical appointments), development and revision of patient care policies and treatment protocols, in collaboration with CSND leadership.
Provides clinical leadership by assessing, implementing and evaluating the care provided to patients and families in the program.
Develops, promulgates, and fosters the implementation of process and outcomes standards for the delivery of patient care relevant to area of practice.
Maintains awareness of research protocols applicable to patient population, ensures knowledge of staff, protects rights of patients, and participates in assessment/evaluation of outcomes.
Provide key support to the CSND clinical, educational and research workgroups as appropriate.
Coordinating clinical care, quality assessment, education planning (professional and community) and outcome measurement. Assist CSND leadership in developing and maintaining strategic relationships with internal NYU Langone Health staff and physicians and external organizations/individuals that will assist in generating interest in the CSND and program referrals to physicians and clinical service areas represented by the CSND.
Following up with patient/caregiver to ensure that care plans are understood.
Ensures and participates in providing ongoing patient/family education consistent with the individual needs of the patient/family.
Communicates significant patient care issues, family concerns, safety and critical events to seeking consultation and assistance when necessary. Identifies, initiates investigation and proactively responds on an immediate basis to potential and actual risk management issues per standards and institutional policies.
Act as a staff resource and professional role model for CSND clinical coordination by incorporating expertise and related experience.
Act as a liaison between clinical and administrative teams to ensure clear communication, future planning and resource need identification.
Build and expand relationships throughout the referral network.
Promotes patient, physician and other consumer satisfaction by facilitating excellent customer service by all members of the health care team.
Collaborate with the CSND researcher committee to identify research recruitment opportunities and possible research projects/collaborations.
Develop and implement patient/caregiver education programs, in collaboration with CSND leadership.
Provide clinical CSND related consultation, as appropriate.
Communicate and work as an effective team with all internal and external customers.
Functions as a respectful member of the health care team. Facilitates an environment conducive to creativity in practice, collaboration with all members of the health care team, and the fostering of collegial relationships. Provides a suitable plan for optimal availability of resources for safe, competent, and effective patient care delivery.
1. Graduate of an accredited Nursing Program. 2. Current Registered Professional Nurse in New York. 3. Baccalaureate degree in Nursing required. 4. At least three years of clinical practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion which addresses patient/family satisfaction. 5. Competence Progressive leadership; demonstrated in a leadership position or demonstrates the potential for leadership competency; demonstrates ability to develop, guide, motivate, nurture, and coach others; participation in service or departmental committees. 6. Evidence of excellent interpersonal skills, effective communication skills and innovative problem solving..
Required Licenses: Registered Nurse License-NYS
Preferred Qualifications: 1. Experience with research 2. Experience in public speaking and presentations 3. Demonstrated ability to facilitate group processes and workflow implementation as a member of an interdisciplinary team 4. Demonstrated experience in heavy interactions with administrators, physicians, clinical staff, community members and patients. 5. Master's degree in Nursing or health-related area preferred. 6. Certification in relevant practice area preferred.
