The WOC Clinician is a Registered Nurse and a member of the care delivery team. The WOC Clinician is responsible to set the standards for the level and quality of care for patients with ostomies, incontinence, wounds, fistulas and skin care. The WOC nurse Clinician has responsibility and authority and accountability for the provision of WOC nursing care to applicable patients. The WOC nurse Clinician provides clinical support and direction to WOC prepared and non WOC prepared nurses within the department. The WOC nurse Clinician manages and provides specialized nursing care for patients and their families through the application of independent judgment, communication and coordination with all team members. The role of the WOC Nurse Clinician encompasses leadership, partnership, and collaboration with other members of the Healthcare Team. The WOC nurse clinician establishes and maintains collaborative relationships with physicians, staff nurse, other health care providers, patients and their families to achieve desired patient outcomes throughout the continuum of care. The WOC Nurse Clinician demonstrates a commitment to the community and to the nursing profession. The WOC Nurse Clinician is responsible for the completion of employee evaluations, selection and hiring of staff, and in performance management of staff in the WOC nursing department.
Actively participates in shared governance at department level and serves on or acts as a resource to hospital-wide councils. Responsible for providing service-based education and clinical expertise for staff including orientation and training. Serves as a leader in clinical nursing practice, identifying patient and staff focused opportunities for improvement and leading the change process. Identifies, educates and supports the psychological and learning needs of staff during change and transition. Applies the Nursing process within the framework of Relationship Based Care to create a healing environment. Formulates goal and a plan of care for applicable patients that involves the patient as a partner and considers the individual needs of the patient in a holistic manner. Practices superior communication skills and is able to articulate and translate the patient's condition to other care providers negotiate and make recommendations for changes in patient care. Communicates the plan of care to the nursing staff, physicians, patient and family, and other members of the health care team as applicable. Documentation of patient care and plan of care is complete and promotes communication between caregivers.

Chairs or participates the WOCN Value Analysis Team, Coordinates and evaluates equipment and products for ostomy and skin care needs for the health system to ensure cost effective quality patient care.
Demonstrates critical thinking in the identification of clinical, social, safety, psychological and spiritual issues for the patient within an episode of care. Conducts stoma site marking according to the accepted standard of stoma marking to ensure proper placement of an abdominal stoma. Maintains ongoing follow-up evaluation for appropriate ostomy and/or skin care patients with needs after discharge from the hospital to provide for continuity of care. Incorporates national professional organizations (NPUAP) and (WOCN) as well as business unit and health system's goals to improve patient quality, safety and satisfaction in daily work Creates a caring and compassionate patient focused experience by building healing relationships with patient and their families as well colleagues. Acts as a resource to colleagues within the system
Participates system wide WOCN meetings and responsible for dissemination of information Participates in continuous learning within the specialty Mentors and role models quest for continuous learning through formal education and the development of others. Seeks opportunities to share expertise with other members of the healthcare team within and beyond the clinical unit/department.
Serves as a highly developed role model of professional nursing practice by serving as a recognized leader in clinical expertise , teacher, specialized resource nurse, and /or evidence based practice in the nursing care of patients with alterations in skin integrity, wounds, ostomies, fistulas and /or incontinence. Consistently gives appropriate feedback to all members of the healthcare team and participates in the peer review process.
Minimum of 3 years nursing experience
Certificate of attendance of a WOCN board certified WOCNEP required
Licensure, Certifications, and Clearances:
Current Pennsylvania licensure as a Registered Professional Nurse
Certification in wound and ostomy from the WOCN required
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
