Job Description Details:
The Lung Nodule Nurse Navigator will serve as the coordinator for the Lung Nodule Program. They will use their clinical nursing expertise to guide patients to informed decision-making. In collaboration with other members of the healthcare team identifies, educates and guides the patient with a lung nodule diagnosis through the healthcare system for timely screening, diagnosis, treatment and follow up across the continuum. Functions in the spirit of teamwork and is accountable for the coordination of patient care and supportive services utilizing the nursing process within the standards, policies, procedures, and guidelines of the organization.
Manages a select patient case load to coordinate care and treatment plans.
Coordinates Lung Nodule Clinic with the priority being patient care needs.
Assures development and documentation of treatment plan for lung nodule patients.
Maintains tracking database to monitor compliance to treatment plan.
Tracks metrics for Lung Nodule Program & prepares reports.
Renders clinical expertise when needed.
Communicates pertinent clinical information to appropriate members of the healthcare team regarding patient/family desired outcomes and treatment choices.
Functions as a resource and role model within the community and Mount Carmel.
Graduate from school of nursing with a BSN required
Current licensure to practice as a Registered Nurse in the State of Ohio
Minimum of 3-5 years clinical nursing experience with experience in case management, disease management, or medical social work preferred
