Position Summary: We have an exciting opportunity to join our team as a Coord-Practice Outreach-NYUPN.
In this role, the successful candidate Plans, coordinates and executes practice outreach activities and communication strategies of the NYUPN Clinically Integrated Network. Communication outreach will include, but is not limited to, orientation and interpretation of policies and guidelines to improve the quality of patient care, cost efficiency and network access. Establish an effective relationship with the NYUPN Practice Outreach Manager, and other departments, to address key issues, identify areas of opportunity, and improve outreach strategies. Responsible for facilitating communication and building relationships with physician partners, practice administrators, managers and front-line staff, to achieve the goals of the NYUPN CIN..

Is willing to adjust personal schedule to accommodate departmental and NYUPN practice needs
Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of duties.
Resolves issues identified by network physicians related to referral access, out of network costs, or reports by communicating with the appropriate internal department.
Recommends potential physician partners for engagement in the NYUPN quality committees, work groups and related performance improvement initiatives.
Responds and follows up to client (physician) suggestions for improving measurement reporting including performance dashboards and NYUPN portal. Clearly communicates recommendations to Practice Outreach Manager, records and tracks all such issues to resolution.
Analyzes performance measurement data and recommends practical solutions for improvement in areas of quality, cost efficiency and network access when applicable.
Cultivates relationships with client membership, their office/practice staff, and various internal/external constituents.
Maintains frequent contact with NYUPN physician practices in between on-site visits via telephone, video conferencing or other means to maintain communications and relationships.
Helps maintain relationships between NYUPN CIN, UPN membership, and Faculty Group Practice department to educate practices about operational policies and procedures impacting the operations of the office/practice.
Makes regular on-site visits to NYUPN physician practices for the purpose of communicating performance measurement results and facilitating quality improvement initiatives and interventions designed to achieve positive results.
Responsible for timely and accurate communication and delivery of physician-level clinical performance measures of the NYUPN, including HEDIS, Cost-of-Care, Patient Experience and others in accordance with standardized measurement specifications.
Educates physician practices about the terms and conditions of the contracts between the CIN and each Managed Care plan and assist in identifying needed changes to maximize contract compliance.
Minimum Qualifications: To qualify you must have a Bachelors Degree or a minimum of three years in a physician office, hospital or managed care environment. Proficient in the use of Excel and Word. Knowledge of claims processing systems and guidelines. Ability to analyze problems, draw relevant conclusions, and devise appropriate courses of action. Excellent written communication and oral presentation skills. Exceptional relationship and organizational skills.
Preferred Qualifications: Masters Degree in Health Care, Public Health Administration or Business Administration. Experience in an HMO, IPA or large group practice setting. Superior customer service skills.
