Provides professional nursing care under supervision of a Registered Nurse.
Minimum Education Requirements
High School Diploma or GED
LPN Program per state guidelines.
Minimum Experience Requirements
2 years recent nursing experience.
Prefer one year of prior hospice, home care or LTC experience.
Ability to demonstrate competency in the provision of individual care of patients based on the patients' assessed age and developmental needs.
Current Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) preferred
Current Colorado Licensed Practical Nurse (LPN)
Current Colorado Intravenous Certification (IV and C-IV)
Current CO Drivers license and auto insurance (Field staff )
Displays critical thinking skills by appropriately utilizing data obtained in evaluations to assist RN in developing the plan of care.
Implements the plan of care.
Demonstrates clinical competence appropriate to discipline.
Provides patient and family teaching on an ongoing basis, based on assessed needs, using appropriate resources.
Demonstrates knowledge and applies Standards of Care and Practice, policies and procedures.
Demonstrates a appropriate utilization of supplies, equipment and resources.
Utilizes additional disciplines appropriately.
Collaborative Practice – 25%
Written and verbal communication with interdisciplinary team per care coordination policy.
Participates in interdisciplinary meetings as appropriate.
Implements goals and a plan of care in collaboration with the patient, family and health care team members.
Ensures continuity of care through reporting appropriate patient/family information to other team members.
Appropriately utilizes agency communication system (email, MVW, pagers, voice mail, mailboxes, communication books, bulletin boards)
Informatics/Technology – 20%
Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care setting.
Thorough and confidential maintenance of patient charts and records.
Document ongoing status, interventions, patient response and outcomes in accurate, timely manner.
Use information technology to communicate, manage knowledge, mitigate error, and support decision-making.
Documentation – 15%
Submits complete and accurate paperwork per agency timelines.
Daily notes reflect skilled care and are complete.
Medication Profile is complete and reflects teaching.
Coordinates patient's needs with supervising RN obtaining and transcribing physician's orders as required.
Professional Accountability – 15%
Maintains productivity per FTE status.
Participates on committees and work teams.
Participates in the education of orientees, ancillary staff and students.
Demonstrates knowledge of QI process and actively participates in and promotes QI activities.
Rotates on-call/staffing responsibilities as designated.
Participates as required in safety training and understands and follows agency policies and procedures in all safety areas.
Performs job functions/duties in a safety conscious manner as required by governing rules and regulations and reports any potential safety issues to the appropriate person.
Medium Work - exert/lift up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly.
