The Clinical Risk Coordinator reports to the V.P. Risk Management and works collaboratively with the General Counsel as well as the risk management team in the development, implementation and review of clinical risk management frameworks to ensure the facilitation of best practice across all parts of the organization. The main focus will be on establishing and improving systems and processes to monitor, follow up and reduce the incidence of adverse events and enhance patient safety.
This position requires mature, high level communication and negotiation skills, particularly in dealing with sensitive clinical information and adverse clinical events.
Risk Management and Patient Safety Promote an organizational culture that allows safety issues to be openly reported, discussed and addressed. Lead and communicate 'best practice' in clinical risk management.
Ensure the day-to-day review of the incident reporting system and follow up of potentially compensable events. Assist with the implementation of risk assessments and documentation review regarding significant care issues. Identify and liaise directly with patients and family members regarding potentially compensable events to help eliminate litigation matters and provide resolution where appropriate benefitting both the provider and the patient/family. Review medical records, facilitate litigation production and prepare records for litigation and medical malpractice matters. Review incident reports, follow up and assist with Root Cause Analyses (RCA) Identify potentially compensable events and coordinate with company and operations staff. Provide support to the clinical operations team in risk management areas Ensure that recommendations are monitored and tracked to completion Liaise with and provide assistance to clinical operations team relevant to patient safety and clinical risk management projects, including skin issues, falls and medication errors. Assist with the development and coordination of the risk management education program to promote organizational awareness of the principles of clinical risk management. Conduct clinical documentation audits and reviews, as requested, as part of the Avamere Compliance Program.
Education, Experience and Training Licensed nurse, LPN or higher. Ability to be empathetic but firm with people who perceive that they have been wronged. Good negotiating skills. Clinical experience in acute patient care, long term care, home health or hospice patient care in situations where clinical risk and outcome review are applicable. Demonstrated understanding of clinical governance, clinical risk management and quality improvement concepts with particular emphasis on systems improvement, continuous quality improvement and organizational change. Knowledge and understanding of systems within a complex healthcare organization. High level communication and interpersonal skills with the capacity to work consultatively and collaboratively with staff at all levels including senior clinical personnel and managers. Excellent writing and analytical skills including a demonstrated ability to prepare reports using clear, concise language; organize information into a logical sequence and include content appropriate for the purpose and audience. Highly developed work organization and project management skills with the capacity to manage competing priorities and respond to emerging needs, work towards specified targets and deadlines and pay attention to detail. Ability to self-direct and initiate without constant supervision. Competence in document preparation using MS Word, Power Point and Excel. Knowledge and understanding of processes for dealing with medicolegal matters.
