Work collaboratively as a member of the interdisciplinary team to provide excellent clinical care
Work to assure a smooth transition of patient care and education between inpatient and outpatient care
Screening, diagnosis, disease process and medical management
Completes timely and accurate documentation in Epic
Communicate to all referring providers and primary care practitioners are needed
Participate in patient and family education in both the inpatient and outpatient setting
Clarification of the medical plan of care and assisting the team to assure all communication loops are closed
Providing telephone advice and follow-up phone calls as needed regarding hospitalization, clinic visits and community care
Ongoing orientation and education of residents to the management of pediatric palliative care patients and families
Participate in teaching, conferences, rounds, and the education of medical students, residents, fellows, nursing and other staff
Provide pediatric palliative care education to the local, national and international communities
Professional development through attendance and/or participation at national/regional conferences
Scholarly Activities: Engages in professional activities related to interest and expertise and participates in scholarly productivity that meets the schools promotion and tenure requirements by faculty rank. For faculty in the practice mission, demonstrates competence in an area of clinical expertise and engages in inquiry and scholarship that contributes to the practice of clinical practice. For faculty in the research mission, advances a program of research, prepares grants and seeks funding, and engages with other missions to foster inquiry within the SON.
Education Activities: Provides effective didactic instruction based on teaching assignment. Uses appropriate instructional technologies to ensure that course outcomes are met. Participates collaboratively in the development, implementation, and evaluation of curriculum. Mentors and advises students and staff. At the doctoral level, supervises students in dissertation and/or clinical inquiry projects.
Service Activities: These activities could include faculty meetings, committee membership, professional activities and other responsibilities as assigned by the Program or Academic Director. Participates in faculty governance and provides leadership in local, state, or national organizations.
Administrative Activities: As negotiated with Program Director.
Other Activities:The appointee shall provide services as assigned by the supervisor in furtherance of the universitys missions and goals of teaching, research, patient care, outreach and public service.
Education: Minimum of a Masters Degree as required by specific Advanced Practice Provider Provide role. A doctoral degree is preferred.
Preferred: Pediatric Acute OR Primary Care National Certification, ELNEC-Peds, PALS
Experience: Minimum of one year clinical practice experience as an acute or primary care pediatric nurse practitioner, pediatric palliative care experience preferred, teaching experience at the APN level preferred
