Position Summary: We have an exciting opportunity to join our team as a Case Manager- Buprenorphine Coordinator- Family Health Centers
In this role, the successful candidate , will serve as the Buprenorphine Coordinator for the Family Health Centers. Family Health Centers at NYU Langone serve a large community of linguistically and culturally diverse patients in the South Brooklyn area, in 9 different clinics at a federally qualified health centers. The Nurse Case Manager Buprenorphine Coordinator will have the opportunity to monitor and expand evidence-based treatment for opioid use disorder within our FHC network. The Nurse Case Manager Buprenorphine Coordinator will work in close partnership with the buprenorphine addiction champion physician to coordinate and monitor prescription of buprenorphine for patient treatment. A goal for the three-year funded position is to expand buprenorphine treatment program from its current one site into at least two other sites within the Family Health Centers network. The Nurse Case Manager Buprenorphine Coordinator will play a central role working directly with clients, delivering person-centered care and maintaining relationships with clients, pharmacies and local addiction treatment programs.
- Coordinate the expansion of office based opioid treatment (OBOT) program within two new FHC sites.
- Collaborate with the buprenorphine champion physician to expand opioid treatment program
- Provide telephone-based care management to patients with opioid use disorder, including urgent appointment scheduling, prior authorizations and care coordination.
- Assist newly waived buprenorphine physicians with care management
- Maintain a registry of patients in treatment, and monitor their health
- Develop and maintain relationships with other local addiction treatment facilities
- Assist patients with access to clinicians and medication
- Prepare grant reporting documents for submission to the city each month
Minimum Qualifications: To qualify you must have : - B.S. in Nursing - NY State RN license required - Experience or interest in working with patients with opioid use disorder. - Experience with motivational interviewing preferred.
Preferred Qualifications: - Bilingual Spanish or Arabic preferred but not required - Facility with Microsoft Office including MS Word and MS Excel - Facility with EPIC Electronic Medical Records
