We have an exciting opportunity to join our team as a Social Worker in the Social Work Services Department. To qualify a Master’s Degree in Social Work (or its equivalent from a social work program that is registered by the New York State Education Department as qualifying for licensure), from a recognized college or university program accredit by the CSWE. New York licensure as a Licensed Clinical Social Worker (LCSW) or a Licensed Master Social Worker (LMSW) or a New York State Education Department – issued limited permit required. Minimum of three (3) years of post LMSW experience as a social worker in the health care field preferred. Must complete coursework or training in the identification and reporting of child abuse in accordance with Section 6507(3) (a) of the Education Law. Excellent interpersonal and communication skills required.
In this role, the successful candidate under the supervision of a Social Work Supervisor or Director, provides psychosocial assessment and counseling services, including discharge planning to patients as assigned by the Department of Social Work at a level of independence requiring minimal supervision. Additional responsibilities may include program development, task supervision, group work, clinical supervision of social work interns, orientation of new staff, and chairing of departmental committees. Qualified candidates must be able to effectively communicate with all levels of the organization.
Provides timely and effective psychosocial assessments and evaluations as required.Uses clinical knowledge of psychosocial dynamics and interviewing techniques to gather social history, define problems and develop treatment plan.Coordinates interdisciplinary information and recommendations into social work treatment plan.Establishes post-hospital/discharge plan in a timely manner.
Implements plan for psychosocial interventions and post hospital care/discharge plan according to patient need and at a senior social worker level.Uses appropriate interventions according to patient/family dynamics and needs considering a variety of approaches and with minimal supervisionIs knowledgeable about resources and public entitlements and incorporates this information in discharge plan.Develops a therapeutic and supportive relationship with the patient and family and maintains a proper perspective between personal and professional matters.Uses professional judgment and creativity in seeking and presenting alternative or innovative solutions to problems encountered in delivery of social work services.Provides information and education to patient/family pertinent to patient needs and discharge plan.
Documents all social work activities in a timely manner according to departmental protocols.Maintains charting on HIS system or outpatient records in a timely manner consistent with departmental protocols.Records are clear, legible and concise.Documents assessment of cultural and spiritual needs.Contributes to patient/family education and documents accordingly.For Rusk, enters admitting and discharge FIM data in a timely manner and documents barriers to life participation.
Performs clinical duties in an independent manner, with minimal supervision required.Demonstrates the knowledge and skill in counseling techniques necessary to provide care appropriate to assignment at a level requiring minimal supervision.Demonstrates ability to work independently, taking responsibility and initiative in providing appropriate and effective social work services.Demonstrates sound professional judgment in the management of cases.Organizes work to achieve efficiency and complete assignments.Perceives the needs of the service unit and the department and offers suggestions or recommendations for positive change.
Carriers out special professional projects according to the needs of the department, including volunteer programs, psycho-educational and group programs and others as required.Recommends, initiates and develops projects and programs as appropriate for patient support services and in accordance with departmental recommendations.Demonstrates flexibility in adapting to changes and shifting departmental priorities.Provides data and reports on programs as needed. Organizes work to achieve efficiency and complete assignments.
Demonstrates skill in presentation of psychosocial information in interdisciplinary rounds, meetings and Rusk evaluation conferences and works effectively in multi/interdisciplinary teams.Participates actively and effectively in the presentation of psychosocial/discharge information at team rounds and evaluation conferences.Contributes to staff/hospital meetings by serving regularly and productively Collaborates in formulating and coordinating discharge plan.
Participates as a field instructor on departmental field placement program for schools of social work and/or provides clinical supervision to designated staff.Instructs students/staff in accordance with established departmental procedures.Monitors the clinical performance of students/staff and provides clear and effective feedback in a timely and appropriate manner .Demonstrates skill in teaching and mentoring.Instructs student/staff in accordance with established departmental procedures, including providing orientation and necessary instructional hours .
