About This Role
The Advance Practice Provider (APP) provides diagnostic and therapeutic health care to patients through assessment, diagnosis, and treatment in the ambulatory care setting in collaboration with a designated PHMG physician. Obtains, evaluates, interprets and develops a plan of care based on lab results. This position offers the APP the opportunity to manage a panel of patients alongside a primary care provider.
The Advanced Practice Provider is responsible for assessment, diagnosis and treatment of patients aged birth to geriatric. Conditions managed include both acute and chronic illnesses with management of co morbidities in the ambulatory care setting.
Obtains medical histories and ensures relevant documentation in accordance with PHMG standards, the state of Wisconsin, PHMG Scope of Practice and collaborative agreement with PHMG physician.
Interprets and evaluates patient care data in order to determine management and treatment of diagnoses in accordance with evidence based practice, current clinical guidelines and medical processes. Orders and interprets diagnostic studies in the field of trained clinical specialty. Performs minor surgeries and routine procedures.
Encourages patient involvement in care through counseling, instructing and educating. Promotes active discussion with patient regarding plan of care, treatment options, care outcomes, preventative health measures and self-care skills.
Provides coordination of referral services including managing a variety of ancillary services, community resources and preoperative plans.
Prescribes, recommends and regulates both pharmacologic and non-pharmacologic therapies including controlled substances, as warranted for patient specific conditions.
Adjusts patient plan of care by monitoring treatment plans and patient outcomes.
Participates in all quality care measurements including WCHQ, PHS-ACO and other quality driven programs.
Consults with physician as appropriate.
Documents patient encounters, assessments and communication in timely manner.
Provides clinical direction/delegation to nursing staff regarding interventions identified in the plan of care. Follows through with nursing staff to ensure patient receives recommended care.
Takes an active role in identifying and creating opportunities to educate, motivate and collaborate with staff and students in their professional development.
Participates in APP governance with adherence to standard pillars of practice as defined by individual governance structures. Engage in quality related process improvements.
Minimum Education Level- Master's degree in Nursing
Bachelor's degree in Physician Assistant
Minimum Clinical Experience- A minimum of 3-5 years full time clinical management of a patient population years or 6,000 hours of direct patient care is preferred.
Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
Certification as an Advanced Practice Nurse Prescriber by the Wisconsin State Board of Nursing
Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
Physician Assistant license issued by the state in which the caregiver practices.
Physician Assistant (PA-C) certification issued by the National Commission on Certification of Physician Assistants (NCCPA).
American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required upon hire.
DEA registration is required or must be eligible to obtain.
Attend mandatory education requirements for PHC.
Demonstrated ability to work as a licensed, independent practitioner within a collaborative practice agreement as directed under Wisconsin state law for Advanced Practice Providers. Demonstrated ability to work as an effective member of the health care team.
Complete Continuing Education required in maintaining state licensure and certification.
