Secures contact names/addresses/telephone numbers/e-Mail addresses and helps create and maintain outreach database.
Develops key contacts with community and government-based agencies.
Through connections made with community- and government-based organizations, reaches out to low-income and underserved members of Latino/Hispanic populations, and other low-income and underserved populations.
Promotes and supports health education and prevention efforts.
Promotes and helps provide education regarding health insurance programs available.
Participates in networking events and other community-focused events.
Ensures phone calls and requests for information are handled promptly and courteously.
Ability to work evening and weekend hours as needed
Indicates an interest in personal growth and VNA mission/goals and participates in staff meeting/in-services.
Familiarizes and complies with all VNA policies and procedures.
Meets standards of behavior expectations.
Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.

Bilingual in English/Spanish oral and written word required. Able to translate effectively.
Effective organization and communication skills, including neat and legible hand writing/printing.
Excellent computer skills preferred. Good working knowledge of Microsoft Office software, especially Word and Excel.
Previous health care case management or social work experience preferred.
Holds a current Illinois state D-License; has access to an automobile
