We have an exciting opportunity to join our team as a House Calls Nurse Practitioner in the Primary Care Plus program.

In this role, the successful candidate will provide primary home care for high cost, high utilizing Medicaid and Medicare patients. This position requires home care visits in regions including, but not limited to, Kings County.
Conducts in-home assessments on assigned high cost members, including past medical history, review of symptoms, physical examination, medication review, social determinants of health and depression screening
Responsible for checking vitals, conducting a physical exam that includes all necessary components for required quality of care measures (for example: monofilament exams, peak flow measure [handheld spirometry] and get up and go tests)
Identifies patients who would benefit from telephonic RN care management as well as in-home care coordination through the Brooklyn Health Home Program
Formulates a list of current and past medical conditions using clinical knowledge, judgment, and the findings of assessment
Communicates directly with the primary care provider of record or facilitates a relationship with a primary care provider within NYU Langone Health
Works with primary care provider to communicate current gaps in care
Provides health education to patients as needed regarding disease management, medication adherence, and other categories as needed
Complies with all HIPAA regulations and maintains security of protected health information (PHI)
NYS Nurse Practitioner license (or eligible to obtain)
Adult, Geriatric, Family or Acute Care Nurse Practitioner
Board Certification from one of the following: ANCC, AANP, or AACN
Spanish speaking proficiency required
At least one (1) year of related clinical experience in highest level of education OR three (3) years of experience as a Registered Nurse
Access to reliable transportation that will enable travel to client and/or patient sites within designated area
Ability to obtain DEA licensure/prescriptive authority post-hire
Ability to spend at least 1 hour with a member in their home, which may be in an inner-city environment in Brooklyn or in another part of New York City, in the presence of pets or individuals who are tobacco users
Ability to lift a 30 pound bag in and out of car
Ability to navigate stairs and a variety of dwelling conditions and configurations
Ability to work autonomously and extensive travel will be required
Provider should be comfortable working with geriatric patients who have a variety of health complications
Experience in Family Medicine, Gerontology, Cardiology, Internal Medicine or Endocrinology
Bilingual Arabic or Fujianese speaking
Home care or home visit experience
