Position Summary: We have an exciting opportunity to join our team as a Inpatient Rehab PPS Coord.
In this role, the successful candidate coordinates the patient care, training programs, and oversees all aspects of data collection of the IRF-PAI within the Department of Rehabilitation Medicine. Responsibilities: Demonstrate a leadership role in the development of specialized clinical expertise, communication, and teamwork. Education and training of all staff in areas of expertise to further enhance the departments clinical needs. Develops material which will enhance the quality and scope of the care rendered to the patient, (i.e. pamphlets, video tapes, home programs), in collaboration with other members of the team. Develops resource material to be utilized by staff members (i.e., suggests journals for library, accesses research articles, etc.).
PROGRAM COORDINATION: Ensures accurate data collection and reporting-Verifies that all data is accurate and collected and documented in compliance with the CMS PPS regulations. Collects information from clinicians via HIS, paper forms, and verbal exchanges. Monitors back-up documentation in medical charts. Prepares information for encoding into software. Alerts medical Attendings, medical residents, clinical supervisors and nurse managers when information is not completed on time. Monitors length of stay for all patients and keeps medical attending appraised of expected/average length of stay for specific Case Mix Groups. Attends eval/Re-eval conferences as necessary. Reviews demographic information and records ICD-10 codes based on information provided by medical attendings.
PROGRAM DEVELOPMENT/LEADERSHIP: Contributes to the development of systems for collecting and analyzing data relevant to IRF-PPS.
PERFORMANCE IMPROVEMENT: Assists in developing and monitoring patient outcomes per clinical indicators established for purposes of quality and performance improvement. Participates in PPS-related performance improvement teams, Medical Center committees, and other activities as appropriate.
Collects patient healthcare data systematically and on an ongoing basis using appropriate assessment techniques and instruments.
Responsible for leading and coordinating Department of Rehabilitation Medicine performance improvement projects; developing and disseminating reports to staff within the Medical Center as needed.
Performs other duties as assigned.
Minimum Qualifications: To qualify you must have a Masters Degree in Occupational Therapy, Physical Therapy, Speech Language Pathology or Nursing with at least 3 years of experience in inpatient rehabilitation, or a nurse documentation reviewer. Must be computer literate, e.g., Microsoft Word and Excel. Must have good communication and organizational, managerial skills and be able to work in a team setting.
Preferred Qualifications: Licensed rehabilitation professional (OT, PT, SLP, RN), with experience with ICD-10 coding, functional assessment and outcome management, chart documentation, Medicare reimbursement issues, case management, compliance, and data collection. Heavy interaction with Physicians and clinical staff
