This position is responsible for providing a range of direct services,including individual and family therapeutic services to patients, including assessments, treatment plans and community paramedicine program graduation planning. The position requires a comprehensive knowledge of all functions of the Community Paramedicine Division, along with a considerable knowledge base of community resources.
This position will address social barriers and gaps of enrolled community paramedic patients and systematically formulate appropriate plan(s) of care by incorporating social, cultural/ethnic, emotional and spiritual aspects of care.
Normal indoor/office environment, extensive phone work and coordination of division patients. Indoor/outdoor environment, which may be noisy, uncomfortable, and unkempt. Some stress is anticipated in association with performing certain responsibilities, primarily due to deadlines, irregular workflow and the nature of clinical social work. Occasional exposure to communicable diseases when working in a healthcare setting.

Participates in the patient-centered team approach established within the division for addressing patient needs.
Maintain patient privacy and keep all protected health information (PHI) secure.
Maintain regulatory responsibilities under HIPAA
Performs short-term counseling and crisis intervention for division. Facilitate the social service triage, assessment, and care coordination needs of assigned patients of the division.
Maintains a case load which consists of reviews, assessments, referrals and follow-ups based on the needs of the division.
Addresses social barriers and gaps of enrolled community paramedic patients, and align patients with appropriate community based resources.
Authors and completes care plans/reports for division patients.
Systematically gathers pertinent psychosocial data for high risk patients to formulate appropriate plan of care by incorporating social, cultural/ethnic, emotional and spiritual aspects of care.
Addresses referrals from providers including referrals via warm hand-offs.
Communicates with team members regarding patient needs and interventions utilized; attends team meetings, huddles, and/or rounds.
Maintains weekly scheduled appointments.
Collaboratively works with multidisciplinary team members including external partners.
Completes required documentation and paperwork including biophsychosocial assessments, treatment planning and implementation in a timely manner.
Demonstrates cultural competency and commitment to patients.
Works effectively with case managers, clients, and other hospital/physician administration officials to schedule in-home visits.
Provides clinical supervision to social work student interns assigned to the Community Paramedicine Division
Performs other related work (including weather or other extreme emergency duties) as required.
