The Midwest Shelter for Homeless Veterans has an opportunity for experienced Case Managers to make a difference, impact, support and serve our veterans in a key role.
Reporting to the Supportive Services Vet Families program manager, MSHV has openings for experienced Case Managers. This is a great opportunity for someone who is compassionate, team oriented, detail oriented, self-directed, process oriented, flexible, and has the knowledge and experience with Vets. The Case Manager will be flexible in travel and work collaboratively to cover 7 counties.
Coordinate services and activities to assist veterans who are at-risk of homelessness or who are literally homeless. Also responsible for conducting outreach, household assessments, housing stability plans, regular home visits, and direct services to support housing stability. Provides linkage and coordination for mainstream and VA specific benefits, and collaborate with households to access appropriate community resources.
Key Responsibilities included but are not limited to:
SSVF Client Services. Conducts screening and assessment for literally homeless veterans and their families.
Conduct regular outreach to emergency shelters, Veterans Assistance Commissions, VSOs, VA Medical Centers, etc. to identify and engage literally homeless veterans.
Provides assessment, case planning, housing advocacy, tenant education, and case management/ housing stabilization services to families accepted into the SSVF Program.
Assists clients with obtaining mainstream and veteran specific benefits.
Must be able to accurately enter client data and information into an established HMIS system.
Works with MSHV SSVF Team Lead as needed to cultivate relationships with current and potential landlords as required to maximize units available to families.
Provides aftercare services as needed to families who have become stabilized in housing and exited the program. Prepares and maintains accurate confidential case records. Represent MSHV to local, state, and national organizations as assigned.
Network, when appropriate, with the community at large, depending on client needs. Advocate for clients and make appropriate professional contacts.
Regularly demonstrate ability to effectively manage multiple tasks at the same time due to the often urgent needs of clients served and the hectic nature of the job.
Utilize excellent communication and problem-solving skills.
Qualifications and Preferred Skills:
Bachelor’s Degree in Social Work or related Human Services field (Master’s Degree is preferred), and at least 1 year experience working in the “field”.
Previous experience in working with homeless individuals and families, particularly chronic a plus.
At least one year of working in a program serving veterans is preferred. Veteran status a plus. Housing First/ Harm Reduction approach experience.
Excellent assessment and decision-making skills. Excellent organizational skills and the ability to manage multiple projects. Ability to handle confidential information appropriately.
Competent in Microsoft Office applications including Word, Excel, Outlook and PowerPoint.
Ability to work independently and as part of a team.
Must provide own transportation; frequent local travel to off-site meetings and activities.
Must be available for occasional evening/weekend hours as events dictate.
