We have an exciting opportunity to join our team as a Nurse Practitioner - HIV Specialist/ Primary Care (Ryan White - Early Intervention Grant) located in the Flatbush neighborhood in Brooklyn.
In this role, the successful candidate will provide both HIV care and primary care. Clinical responsibilities include, but are not limited to, performing initial physical examinations, patient care review, coordination of patient care, and appropriate clinical charting and documentation. In addition, during one dedicated weekly session of protected time, the incumbent will be responsible for collaborating with the case manager to outreach and develop relationships with local community organizations, to increase awareness of HIV prevention and care services provided by the FHC.
Coordinate with HIV program team, including case managers, patient navigators, and prevention program staff, to ensure that patients’ needs are appropriately met, and that messaging to patients and community members is consistent across the HIV program
During dedicated protected time, outreach to community organizations on behalf of the HIV program, including making initial contact with agency staff, meeting with agency leadership, and/or offering educational sessions at agencies as appropriate
Participate in case conferences, viremia rounds, and HIV quality committee meetings to identify and address gaps in care
Participate in marketing and promoting the practice and mission of FHC by making public and community presentations to various boards, councils, civic and community groups and other legal bodies
Coordinate and provide oversight for the HIV hotline for newly diagnosed patients within the LFHC network. Monitor hotline notification system to ensure quality assurance and timely linkage to care
Master's degree as a Nurse Practitioner
NYS NP Certification (or eligible to obtain)
NYS Nurse Practitioner (or eligible to obtain)
At least one year of experience as a Nurse Practitioner
Previous experience providing HIV care in a clinical setting
Compassion and desire to work with underserved communities
DEA license is required up on hire
Board Certified Family NP
AAHIVS certification highly preferred
Fluency in Spanish and/or Creole preferred
