Job Summary The purpose of this position is to provide clinical services to adult Primary Care patients with behavioral health disorders as part of an interdisciplinary team utilizing a Collaborative Care Model. This will occur under the supervision of the Langley Porter Hospital & Clinics Associate Medical Director of Integrated and Collaborative Care. This clinician will provide assessment, develop treatment plans and perform brief therapeutic interventions, including both individual and group evidenced-based psychotherapy. Case Management services will be performed utilizing a patient tracking system to maintain a clinical registry of active patients. When appropriate, assists patients in applying for and obtaining other social services while providing a safe and supportive environment and adhering to laws regarding confidentiality and reporting requirements. Activities may include responding to potential program referrals, completion of intake documents, crisis intervention, case management, patient advocacy. The LCSW maintains accurate detailed documentation in an electronic medical record system. Collaborates closely with Behavioral Health and Primary Care providers; make referrals to other community and mental health agencies when indicated. Training will be available to learn the use of web-based registry and therapies utilized in the Collaborative Care Model, such as Problem Solving Therapy and Behavioral Activation. Langley Porter Psychiatric Hospital & Clinics and UCSF Medical Center requires all Advanced Health Professionals (AHP) to be credentialed through its Office of Medical Affairs & Governances (OMAG) in order to practice and function in their clinical role. This applies to both adult and pediatric AHPs in the inpatient and outpatient clinical settings at all Langley Porter and UCSF Medical Center sites. Credentialing and approval of the Standardized Procedures takes between 60-90 days and must be completed prior to the first working day. Required Qualifications
