BEDS Plus Care provides emergency overnight shelter, supportive housing and stabilization services to the homeless and those at imminent risk of homelessness in southwest suburban Cook County. The Family Case Manager will oversee and deliver case management services to families in stabilization services. The case manager will be responsible for developing this program for high-need families and working with the Manager of Prevention Services and the Director of Supportive Housing to stabilize families at risk of homelessness.
KEY RESPONSIBILITIES: Client Case Management
Work cooperatively with Manager of Prevention Services to qualify families for intensive case management services
Consult across program areas (shelter, prevention and housing) to meet the unique needs of homeless families
Ensure client intake process and secondary assessments are consistently and appropriately administered
Lead case management for families in the Lagrange Area Transition Housing Program and Motel Bridge program
Create and document individualized client goal plans for BEDS clients
Connect BEDS clients with government benefits, medical providers, mental health services and housing options
Collaborate with other service providers to connect clients with appropriate services to achieve their goals for up to 24 months
Service as primary point of contact for the area schools in assisting homeless families.
Responsible for reporting client data and outcomes on a regular basis to the Director of Supportive Housing
Responsible for integrity of client files and data entry in HMIS
Excellent written and verbal communication skills
