Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary: We have an exciting opportunity to join our team as a Academic Coach.
The Program Counselor is responsible for providing support for community members and participants at the Family Support Center. This includes intake crisis intervention assessment case management counseling and therapeutic services. The Program Counselor will also play an active role in the Adult Education program and work closely with teachers and students in providing informational workshop to address community needs. Excellent clinical skills required and ability to work with individuals and families in crisis around issues such as domestic violence parenting public benefits medical and school advocacy.
Interviews clients family members and other interested parties to assess needs for social services gathers and evaluates information regarding employment history housing situation physical functioning financial status develops service plans and establishes case files.
Counsels clients on available resources barriers to employment independent living skills and other areas involving defined problems or concerns explains procedures rights and responsibilities.
Identifies and makes referrals to a variety of public and community agencies providing food shelter clothing medical educational and other services schedules client appointments acts as client advocate in obtaining services.
Manages assigned caseload prepares narrative and statistical reports documents correspondence regarding client status documents case files.
Develops service plans and monitors client progress toward delineated objectives counsels clients using a variety of counseling modalities.
Attend staff meetings and other appropriate meetings provided by the agency.
Performs appropriate assessment treatment and crisis intervention
Reports child abuse/neglect when indicated
Minimum Qualifications: To qualify you must have a Bachelors Degree in social work or human services field. 1+ years relevant experience working, ability to plan and manage, knowledge of Social Work methods and practices.
