The Department of Mental Health, Substance Abuse and Developmental Services is currently seeking highly qualified applicants for the position of Program Manager for the Discharge Planning Program at Leesburg Loudoun Center. This full-time position will provide strategic, operational leadership and administrative/clinical supervision to a team of Discharge Planners. The Program Manager position will support the goal of Discharge Planning which is to ensure continuity of quality care between the hospital and the community. In addition, the aim of discharge planning is to reduce hospital length of stay and unplanned readmissions to hospitals, as well as to improve the coordination of services following discharge from hospitals. Discharge Planning Program arranges for aftercare providers, coordinates care with existing providers, and makes referrals to appropriate treatment programs and services. The goal is to meet the needs of the patient and to support goals that were identified during the admission and inpatient stay.
The successful candidate will possess a demonstrated ability to provide the highest quality professional service, maintain exemplary working relationships with individuals, families, and providers, and model best practice in recovery-oriented, person-centered principles. The Program Manager ensures that services are provided, and records are maintained, in accordance with Departmental policy, Virginia Department of Behavioral Health Developmental Services Licensure, Human Rights regulations, and pertinent Medicaid Provider Manuals.
Responsibilities include, but are not limited to, quality assurance activities, program and staff supervision, program and staff development, monitoring of budget and optimizing billing, representing our Department on local, regional, and state collaborations, and oversight of program performance outcomes to influence strategic and operational planning.

Requires a combination of education and experience equivalent to Master's Degree in Counseling, Social Work, Psychology or related Human Services field; and Minimum of 2 years directly related combined experience in discharge planning services, and supervisory/managerial experience. Effective oral and written communication skills and proficiency in electronic health records is required. Work schedule requires the flexibility to respond to emergency management situations. Clinical licensure (i.e., LPC, LCSW, and LMFT) and/or bilingual in Spanish are highly desirable. Employment is contingent on successful completion of a criminal background check, driving record check, TB test, credit check, and successful completion of Department orientation and training requirements.
