Position Summary: We have an exciting opportunity to join our team as a Associate Transplant Coordinator.
In this role, the successful candidate Serves as primary contact person for patients, living donors, and referring physicians for initial referral and assures insurance coverage for transplant at this program.
Intake for potential kidney recipients Obtain demographics, Insurance , PCP, Referring MD and medical records
Verification of Insurance and Authorization/Referral for consult. Verify consult insurance eligibility (via telephone call which varies in time span)Obtains authorization, if necessary (which requires clinicals to be faxed for medical necessity, Verifies if patient requires a referral from PCP and if deductible or copay is required. Update expired or soon to expire authorizations.
Creates new patient medical record in EPIC, checking off FYI flag, insurance information along with all demographics for the patient.
Create new patient medical record chart, with all the demographics, history and all forms that are required for the patients consent.
Mails out new patient packet to the patient with all information for transplant along with forms that may need to be signed by the patient.Mails out new patient packet.

preparing new consults hand out folders for kidney and liver patients. Print consultation letters and Op reports for correction and signature and make copies for patient chart, medical records, binder and scan into velos. Op notes given to the front desk in order for them to be scanned into EPIC.Prepare and distribute the weekly list of the patients that are going to be presented/discussed at the listing meetings.
Minimum Qualifications: To qualify you must have a Bachelor's degree with at least 6 -12 months of comparable experience.
Preferred Qualifications: 2 to 3 years of experience in a physician practice; previous experience working at a Dialysis or Transplant Center; familiar working with kidney or liver disease patients
Qualified candidates must be able to effectively communicate with all levels of the organization.
