The dietitian plans and provides nutritional care for patients.
The dietitian ensures implementation of current State and Federal regulations for an acute care hospital and assisted living facility.
The dietitian ensures implementation of current sanitation regulations.
The dietitian provides education, planning, and documentation for the DSMES program (Diabetes Self-Management Education and Support).
Essential Duties and Responsibilities:
Performs an accurate nutrition assessment within a timely manner of patient's nutritional status. This includes, but is not limited to, obtaining and evaluating nutritional and medical history, laboratory data and test results, current medications, current diet orders and nutrition regime, assessing patient's estimated nutrition needs and areas that may be impacted by nutritional intervention following established protocols from the Academy of Nutrition and Dietitians, the current Diet Manual and FVH policy. Nutrition evaluation to include consideration of cultural/religious needs, physical and mental limitations, and age specific factors for all age patients.
Plans for the delivery of individualized medical nutrition therapy specific to patients needs with a clearly defined goal.
Documents pertinent information via computerized charting.
Establishes and performs ongoing evaluation for each patient's nutritional care.
Makes referrals to other allied health professionals and other agencies as appropriate to assure optimal patient outcomes.
Provides nutrition education and counseling for inpatients and cardiac rehabilitation patients based on physician orders / or assessment of needs.
Ensures continuity of nutrition care through communication with relevant providers.
Functions as an integral member of an interdisciplinary health care team as evidenced by active participation in team conferences, multidisciplinary care planning and ongoing communication with pertinent team members.
Performs DSMES education planning and documentation.
Essential Duties and Responsibilities continued:
Plans, provides and documents medical nutrition therapy for outpatients per medical providers orders.
Performs audits of facility kitchens to assure sanitation regulations are followed.
Meets regularly with Dietary Department Manager, provides guidance, recommends food and nutrition activities to comply with government regulations, submits reports, Comments and recommendations to Dietary Department Manager.
Provides in-services and training as needed/ or as requested for Dietary staff regarding modified diets, food allergies and sanitation.
Provides community education through lectures and acts as a community resource.
