The Adult Intensive Care Coordinator (ICC) will work to ensure Oregon Health Plan members residing in Washington County have access to care coordination regardless of eligibility for specific services. Intensive Care Coordinators may work with either youth or adults to provide needs assessment, connection to service delivery systems, coordination with health and dental providers, psychosocial evaluation and consultation. Using a person-centered approach, care coordinators facilitate interdisciplinary teams, coordinate with partners such as Housing, Corrections, Juvenile Services and law enforcement and maintain accurate and complete records.
The ideal candidate will be a team player that thrives in a fast-paced workplace and who has a passion for working with adults with complex mental health clinical presentations. We are looking for an individual with strong collaboration skills who is good at building and maintaining relationships with system and health plan partners. The candidate must be compassionate, strengths oriented, and a natural problem solver.
A typical way to obtain the knowledge and abilities would be:
A Master's degree in a clinical field related to the diagnosis, etiology and treatment of mental health disorders; AND three (3) years of increasingly responsible mental health experience in assisting individuals with severe and persistent mental illness in accessing needed services and support; AND
Must be certified by the Mental Health and Addictions Certification Board of Oregon as a Qualified Mental Health Professional or hold an approved clinical license and meet the criteria of a Qualified Mental Health Professional as defined in the Oregon Administrative Rules.
