At Stony Brook Medicine, the Neurosciences Navigator provides high quality individualized coordination of care and manages support and advocacy efforts for patients and families. This individual coordinates the delivery of care, pre-surgical testing and aftercare services for Neurosciences patients. Candidate must travel between the hospital and outpatient offices off site. Responsibilities include the planning, organizing, monitoring and evaluation of care processes and workflow within specific neuro areas, as well as support compliance with regulatory bodies and national guidelines. This position helps to support and maintain high quality, evidence-based care through education and training to achieve optimal patient outcomes, and maintains a strong component of patient/family advocacy with day-to-day review and coordination of multidisciplinary services.
Duties of a Navigator may include the following but are not limited to:
Responsible for the coordination of care between the hospital, departments and aftercare providers for patients being scheduled for neurosciences related surgical and non-surgical treatment services.
Acts as a liaison between Neurology, Neurosurgery and the hospital for hospital patients seeking or receiving care.
Coordinates patient scheduling of MRIs and/or other imaging required prior to procedures such as deep brain simulation (DBS) surgery under anesthesia.
Assists in developing and organizing a plan of care with treating physicians, patient and family members, and assist the patient in understanding/navigating their individualized plan.
Carries out various patient care activities within scope of practice (i.e. brief counseling and education) and accurately documents in the patient record according to hospital and department policy.
Coordinates any services or accommodations necessary (e.g. sign language interpreter) to ensure effective communication between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers.
Insures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals.
Accurately collects clinical data on patients and demonstrates competency in performing assigned interventions and reports abnormalities to medical providers.
Triages patient phone calls and troubleshoots both clinical and non-clinical issues such as medication management concerns, pre/post procedure questions, scheduling issues, etc. safely and in accordance with professional standards and the organization's policies and procedures.
Runs and organizes routine support groups for specific patient populations and/or caregivers
Provides real time feedback and assists in performing concurrent chart reviews to ensure medical record documentation is reliable and up to date for all quality process and outcome monitoring.
Participates in updating and revising policies/procedures and standards.
Effectively determines and set priorities to meet patient needs.
Maintains infection control precautions as appropriate and identifies potential infection control problems.
Provides education to members of the interdisciplinary team within the Department relevant to patient navigation and the continuum of care.
Participates in Quality Assurance activities within the Departments of Neurology and Neurosurgery and the hospital.
Participates in annual recertification training, compliance training, and HIPAA training, as well as other mandatory in-service programs.
Plans and participates in community outreach and education activities independently and in collaboration with other department and hospital resources.
Conforms to all hospital and practice policies and regulation governing patient rights and confidentiality of information.
Performs all other related duties as assigned by hospital supervisor/leadership and designated department supervisors.

Required: The individual should have a minimum of a Master's degree in a human service field such as Social Work, Mental Health Counseling, Rehabilitation Counseling, Nursing, Psychology or an equivalent field. A minimum of 3 years' experience working in a position focused on serving those with disabilities in a hospital or community-based setting. Experience with advocacy and care coordination with and for providers, patients/clients and families. The individual should have the ability to coordinate across levels of care and community, and have excellent oral and written communication skills, as well as a flexible customer friendly and pleasant patient-focused presentation.
Preferred: Any license or certification in fields such as social work (LMSW), mental health counseling (LMHC), rehabilitation counseling (CRC), Care Management (CCM) or nursing (RN).

While performing the duties of this job, the employee is regularly required to communicate with patients, office staff and medical providers. The employee must be able to exchange accurate information in these situations.
This position is active and requires the employees to constantly move about in the hospital and office to assist patients, staff and medical providers.
