Care Management Services' mission is to provide a multidimensional, action-oriented and solution-focused approach to assisting students in meeting a variety of needs to enhance the student experience, persistence to graduation and success. This includes assisting students in navigating the college experience and addressing barriers relative to academic progress and personal success.
Through proactive education, connection to resources, direct support and appropriate follow-up, Care Management helps students address challenges, meet unmet needs and empowers them to overcome barriers to their success. Care Management supports the well-being of the individualized student while balancing the health, safety and welfare needs of the campus community.
The Care Manager serves as a key member of the UCARE Team (University Committee for Assistance, Response and Evaluation) and assists in the assessment, intervention, documentation and follow-up of students of concern.
Respond to inquiries and referrals of students of concern; document information and actions.
Promote student success through intervention, support and prevention to address routine and at times, more complex issues and crises.
Assist students in establishing goals, overcoming obstacles, developing skills, expanding support and finding tools and resources to support academic and professional goal achievement.
Connect students with resources, on and off campus, to address personal, academic and well-being needs.
Assist in coordinating well-being checks and provide post-hospitalization or re-entry assistance.
Assist with predictive-analytics by cultivating a sense of belonging, support and encouragement to promote student retention and success.
Educate students on how to navigate complex systems and on relevant policies and procedures.
Provide preventative and supportive programming for high-risk student populations.
Develop and maintain a comprehensive network of community providers and resources.
Establish and maintain working relationships with internal and external stakeholders.
Act as a resource and liaison to members of the University community on matters pertaining to student well-being and students of concern.
Develop and implement outreach and education strategies for faculty and staff to identify and respond to students experiencing distress or barriers.
Monitor student progress from initial contact through resolution by providing consistent follow up to students and referring parties; monitor compliance with student success plans.
Assist with crisis management and critical incident support.
Serve as a member of UCARE (University Committee for Assistance, Response, and Evaluation).
Maintain student records and uphold privacy using discretion with sensitive information.
Gather data, compile information, document and manage caseload in an electronic database and assist in preparing reports.
Keep abreast of federal and state policy changes that impact scope or practice.
Participate in research, campus committees, programs and projects as assigned.
As needed, assume other duties assigned by the Care Manager and Office of the Dean of Students.
A Bachelor's degree in related/relevant area or an equivalent combination of education and experience, and at least two years of experience from which comparable knowledge and skills can be acquired is necessary.
Master's degree in social work, counseling, psychology, higher education or related field.
One-year minimum experience in care or case management or coordination of care services.
Experience in higher education setting.
Awareness of issues affecting high-risk populations within higher education.
