Supervisor, Case Management oversees a team of RN Case Managers working both in the Inpatient and Outpatient setting. The role involves using an interdisciplinary collaborative approach to fulfill responsibilities that include utilization, concurrent review, safe patient discharge, coordination of care, patient education, working to improve patient care at MSK.

Assess patients on a designated service for discharge planning needs at admission and throughout hospital stay, coordinates home care services, placements and transfers to acute/subacute rehabilitation facilities, as well as acute care hospital transfers.
Work with a multi-disciplinary team and acts as a liaison to post-acute care providers, insurance carriers and patients.
Coordinate staffing and provide advice to Case Management staff, in absence of Manager or Director, Case Management.
Oversees and evaluates the activities of 4-6 direct reports which includes daily management of their work.
In collaboration with Director and Manager contributes to annual review and update as needed of departmental policy/procedure manual.
Provide training to new CM staff
Participate in the recruitment, interviewing and hiring of RN Case Managers, along with the Director and Manager.
Performs evaluations of RN Case Managers that report directly to you.
Current NYS Registered Nurse License/Registration
Bachelors degree in nursing (BSN)
4 years of clinical nursing experience
Previous case management, home care, managed care, or UM/CM experience required
Certified Case Manager (CCM) or Accredited Case Manager (ACM) status preferred,
Excellent interpersonal, problem solving and communication skills essential
Strong decision-making and negotiation skills
Knowledge of state and federal regulations that govern health insurance coverage
Knowledge of Microsoft: Outlook, Word, Excel, Power Point, and Internet Explorer
# Hours: 8-hour day shift, Monday Friday.
