University of Vermont Medical Center, Vermont’s academic medical center and founding member of the University of Vermont Health Network, seeks a Director Care Coordination and Clinical Programs.
The Director of Care Coordination and Clinical Programs is a highly visible and strategically important role facilitating healthcare reform efforts statewide and nationally. The Director oversees effective implementation of the organization’s population health model statewide in support of the Triple Aim- providing better care, improved outcomes, and controlling cost growth. S/he provides strategic insight and direction to the ACO and to Network Participants, Collaborators, and stakeholders to align with a changing health care environment. S/he builds trusting collaborative relationships with senior leaders, payers, regulators, and stakeholders, facilitates buy-in to OneCare’s clinical and quality strategies, and supports change management within and across organizations, communities, and the state. The Director supports the effective implementation of the Care Model statewide, identifies opportunities for improvements, and works collaboratively to advance the Model. S/he develops, implements, and evaluates clinical programs and investments to ensure effective care delivery transformation. The Director oversees OneCare’s care coordination investments and strategy and works with medical leadership to continually evolve and improve the program. The Director is responsible for negotiating payer contract terms within his/her areas of expertise and ensuring that deliverables are achieved. S/he manages clinical governance committees and ensures they include representative voices across the diversity of the ACO Network.

Clinical licensure (e.g. medicine, nursing) in the State of Vermont is required. Bachelor’s Degree is the minimum requirement; additional education in healthcare administration, public health, improvement science, or business strongly preferred.
This position requires 10 or more years of varied clinical experience in a healthcare related setting (e.g. ACO, MCO, or health plan; continuum of care or healthcare delivery agency; or quality improvement setting) demonstrating expertise in healthcare reform, population health management, and quality improvement, including seven or more years of progressive leadership experience. Previous experience at the Director Level and experience in network-based organizations transitioning to value-based care is highly desired.
