Under the supervision of the Community Health Program Manager, the Client Engagement Specialist will provide comprehensive and quality home-based services to clients with HIV/AIDS residing in Chicago. In addition to developing relationships with clients, documenting their findings, and reporting, the Client Engagement Specialist will collaborate with the Community Health team and other program staff to ensure the multiple needs of clients will be met.
Duties and Responsibilities:
Develop and implement treatment/service plans for all clients accessing services.
Provide case management on an individual basis to clients [JH1], including linkages to supportive services, treatment and care.
Conduct home visits to clients on a regular basis, maintain a home visit log, and maintain telephone contact as required.
Collaborate closely with other HIV/AIDS program staff and community and mental health staff to provide holistic services for clients.
Manage administrative and program responsibilities related to the project, including writing of progress notes, evaluations, and reports in an accurate and timely manner to meet confidentiality and compliance standards.
Attend professional trainings each year and adhere to all current DRS’ policies and procedures as prescribed by the Agency and funder.
Assist Housing Assistance staff in conducting home visit and enroll clients in benefits programs as needed.
Skills, knowledge, & abilities:
Requires a minimum of a Bachelor’s degree in social service or a healthcare-related field.[JH2] Licensed social workers and certified case managers a plus.
Experience working with HIV/AIDS clients preferred.
Compassion and desire to work with diverse and complex clients.
Excellent writing, speaking, and organizational skills
Ability to quickly establish rapport with clients and handle interpersonal situations professionally.
Excellent observational skills and attention to detail
Ability to multi-task and prioritize.
Accurately document case notes and enter data under tight deadlines.
Work independently and in a team
Proficiency with Microsoft Office and other related applications
Must have a valid driver's license and have daily access to a reliable vehicle
